Most patients have a degree of disability which is manageable by adjustments in lifestyle. Treating vascular risk factors reduces the risk of further problems due to atherosclerosis.<\/p>\n<p>Hardening of the arteries in the legs only leads to serious problems in a small number of patients. However, if hardening of the arteries progresses, then there may be a risk of\u00a0<a href=\"https:\/\/vascular-society.nz\/amputation\/\" target=\"_blank\" rel=\"noreferrer noopener\" data-type=\"page\" data-id=\"39\">amputation<\/a>.<\/p>\n<p>Arterial bypass surgery is indicated in patients with a threatened leg likely to require amputation, if left untreated (critical limb ischaemia). It can also be considered in patients with very severe disabling claudication. Arterial bypass surgery can save the leg or reverse severe disability.\u00a0 The benefits of bypass surgery in patients with life style limiting claudication are more controversial.\u00a0 Many surgeons will advise against such surgery in these circumstances as surgery may precipitate problems as well as solve them.<\/p>\n<p>Arterial bypass surgery is not minor surgery. Operations are frequently prolonged, complex and carry significant risks. A decision to proceed with an operation to improve the blood supply to a leg needs to be taken in a careful and considered way. It will be the right option for many patients. In other patients an amputation may be more appropriate. In some very sick patients, nearing the end of their lives, the compassionate option may be to avoid any type of treatment and to allow the patient to die peacefully.[\/vc_column_text][\/vc_tta_section][vc_tta_section title=&#8221;How will I know if my leg is in danger?&#8221; tab_id=&#8221;1759012899354-da1d8fff-c098c5af-98fa&#8221;][vc_column_text css=&#8221;&#8221;]\n<h2 class=\"wp-block-heading\">How will I know if my leg is in danger?<\/h2>\n<p>It is usually clear when the leg is in danger although an all embracing definition of critical leg ischaemia has been difficult to formulate. Most patients will experience at least one of the following symptoms:<\/p>\n<p><strong>Rest pain<\/strong>\u00a0\u2013 this is a severe, continuous pain felt in the toes and foot. Some patients find that they only experience this pain at night. This is because during the day when the leg is down, gravity can assist the flow of blood. When the leg is elevated at night this gravity assistance disappears. Many patients find that dangling the foot over the edge of the bed can help to ease the pain. The pain is often very severe, and patients will sleep in a chair to avoid the discomfort of elevating the leg. This frequently leads to significant swelling of the leg.<\/p>\n<p><strong>Gangrene<\/strong>\u00a0\u2013 if the blood supply deteriorates to a stage where insufficient blood is available to keep the tissues alive then gangrene can develop, as shown (below right) in the 2nd toe (previous big toe amputation).<\/p>\n<figure id=\"attachment_136\" aria-describedby=\"caption-attachment-136\" style=\"width: 113px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-136\" src=\"https:\/\/cmcdeploytwo.co.nz\/vsoanz\/wp-content\/uploads\/2025\/10\/Dry-gangrene-of-2nd-toe-.jpg\" alt=\"Dry gangrene of 2nd toe\" width=\"113\" height=\"114\" \/><figcaption id=\"caption-attachment-136\" class=\"wp-caption-text\">Dry gangrene of 2nd toe<\/figcaption><\/figure>\n<p>This means that the tissue has died and turns black. If the tissues remain dry (mummified) then sometimes a toe can autoamputate and drop off without surgery. If it becomes wet (infected) then surgery is frequently required.<\/p>\n<p>Ulceration and loss of tissue \u2013 sometimes frank gangrene does not develop but the tissues can ulcerate. In this situation the superficial tissues die and are shed leaving an ulcer crater which will not heal because the blood supply is so poor.<\/p>\n<figure id=\"attachment_135\" aria-describedby=\"caption-attachment-135\" style=\"width: 170px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-135\" src=\"https:\/\/cmcdeploytwo.co.nz\/vsoanz\/wp-content\/uploads\/2025\/10\/Arterial-ulcer-1-1.jpg\" alt=\"Arterial ulcer\" width=\"170\" height=\"136\" \/><figcaption id=\"caption-attachment-135\" class=\"wp-caption-text\">Arterial ulcer<\/figcaption><\/figure>\n<p>This is shown in the picture (below left) in which an ulcer has developed on the outside of the foot due to a poor blood supply.<\/p>\n<p>Severe disabling claudication \u2013 some patients experience none of the above symptoms but they can develop claudication symptoms after a few steps and managing a normal life becomes virtually impossible.[\/vc_column_text][\/vc_tta_section][vc_tta_section title=&#8221;What will happen if I do not have an operation?&#8221; tab_id=&#8221;1759014772115-45b909ba-0338c5af-98fa&#8221;][vc_column_text css=&#8221;&#8221;]\n<h2 class=\"wp-block-heading\">What will happen if I do not have an operation?<\/h2>\n<p>Amputation is the most likely outcome in patients with critical limb ischaemia (ulceration, gangrene, rest pain), who do not have bypass surgery to improve the blood supply to the leg. In patients with critical limb ischaemia, ulceration and gangrene will not resolve without some form of intervention. Improving the blood supply to the leg can allow healing to take place, but the gangrenous areas still need removal to enable healing to take place.<\/p>\n<p>In patients with severe claudication, but no features of critical ischaemia, prediction of outcome can be difficult. Most surgeons would accept that bypass surgery is reasonable in these circumstances, if walking is becoming impossible even in the absence of gangrene, ulceration or rest pain. Without surgery amputation is not inevitable. Many patients with severe claudication will merely continue with their symptoms and they may not deteriorate.<\/p>\n<p>It must be acknowledged that there are patients in whom amputation may be the most appropriate way forward. There are many reasons for this, but the potential benefits of the operation for any individual must always be balanced against the potential risks and complications.[\/vc_column_text][\/vc_tta_section][vc_tta_section title=&#8221;Should I stop smoking before the operation?&#8221; tab_id=&#8221;1759014806183-f54d1997-de0bc5af-98fa&#8221;][vc_column_text css=&#8221;&#8221;]\n<h2 class=\"wp-block-heading\">Should I stop smoking before the operation?<\/h2>\n<p>Every patient with hardening of the arteries should try to stop smoking.\u00a0 Anything less is unlikely to produce any benefit.\u00a0 Continuing to smoke is almost certain to produce a worsening of arterial disease, although this is unlikely to be immediately apparent as the effects are cumulative and take time.<\/p>\n<p>Damage produced by smoking continously over many years cannot be undone by suddenly stopping smoking.\u00a0 However, even stopping smoking for a relatively short period before operation or angioplasty may reduce your risks of the procedure failing or leading to complications.\u00a0 Continued smoking after lower limb bypass surgery results in a threefold increased risk of the bypass graft failing (<a href=\"http:\/\/download.journals.elsevierhealth.com\/pdfs\/journals\/0741-5214\/PIIS0741521405004623.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Willigendael EM et al, 2005<\/a>).[\/vc_column_text][\/vc_tta_section][vc_tta_section title=&#8221;What tests will I have before surgery?&#8221; tab_id=&#8221;1759014827533-6ef99622-00a9c5af-98fa&#8221;][vc_column_text css=&#8221;&#8221;]\n<h2 class=\"wp-block-heading\">What tests will I have before surgery?<\/h2>\n<p>Workup will include detailed history and examination. This will pay particular attention to previous vascular disease, previous bypass surgery, medications and cardiorespiratory function. Angiography will be performed to provide your surgeon with a roadmap of the arteries. This detailed information will tell the surgeon whether bypass surgery is likely to be beneficial and will permit detailed planning of any bypass that is possible.\u00a0 Mapping of the veins in the leg using ultrasound is also often useful as the veins can be used to provide a channel to take blood to the arteries lower down the leg. Sometimes angioplasty can be performed to improve the blood flow to the foot. In patients with severe limb ischaemia this is frequently a less common option as the arterial disease is usually more extensive and will often span 2 or 3 levels. Whether angioplasty is attempted or not will vary from centre to centre depending on the level of expertise in both angioplasty and surgery. Neither is an easy option and both have risks and complications.[\/vc_column_text][\/vc_tta_section][vc_tta_section title=&#8221;How do bypass operations work?&#8221; tab_id=&#8221;1759014855534-cd1bf395-e355c5af-98fa&#8221;][vc_column_text css=&#8221;&#8221;]\n<h2 class=\"wp-block-heading\">How do bypass operations work?<\/h2>\n<p>The essential problem in all patients undergoing bypass surgery to the lower legs is that insufficient blood and hence oxygen is able to reach the tissues of the leg and foot. The tissues are starved of oxygen and this is what leads to pain, gangrene and ulceration. Bypass operations work by improving the blood supply and hence the oxygen supply to the tissues that have been starved of oxygen. To do this the surgeon must take blood from a good artery above the area where the artery is blocked, and take the blood along a tube (the conduit) to a good artery below the blocked segments. There are variations on this principle when blood can be taken from one side of the body to the other (femoro-femoral crossover) or blood is diverted from arteries to the arms down to the legs (axillo-bifemoral grafting).[\/vc_column_text][\/vc_tta_section][vc_tta_section title=&#8221;What operation will be performed?&#8221; tab_id=&#8221;1759014887716-648cb14b-0019c5af-98fa&#8221;][vc_column_text css=&#8221;&#8221;]\n<h2 class=\"wp-block-heading\">What operation will be performed?<\/h2>\n<p><strong>Inflow operations<\/strong>\u00a0Inflow operations are performed to restore blood flow to the top of the leg.\u00a0 When the blood flow is normal to the legs, femoral pulses can be felt around the groin area.\u00a0 If major blood vessels (aorta or iliac arteries) are blocked then the blood flowing into the leg is severely reduced.\u00a0 An inflow operation is required to restore this flow to normal.<\/p>\n<p>There are many operations to restore inflow to the legs, but the commonest are:<\/p>\n<figure id=\"attachment_134\" aria-describedby=\"caption-attachment-134\" style=\"width: 176px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-134\" src=\"https:\/\/cmcdeploytwo.co.nz\/vsoanz\/wp-content\/uploads\/2025\/10\/Aorto-bifemoral-bypass-.jpg\" alt=\"Aorto-bifemoral bypass\" width=\"176\" height=\"361\" srcset=\"https:\/\/cmcdeploytwo.co.nz\/vsoanz\/wp-content\/uploads\/2025\/10\/Aorto-bifemoral-bypass-.jpg 176w, https:\/\/cmcdeploytwo.co.nz\/vsoanz\/wp-content\/uploads\/2025\/10\/Aorto-bifemoral-bypass--146x300.jpg 146w\" sizes=\"auto, (max-width: 176px) 100vw, 176px\" \/><figcaption id=\"caption-attachment-134\" class=\"wp-caption-text\">Aorto-bifemoral bypass<\/figcaption><\/figure>\n<p><strong>Aorto bifemoral grafting<\/strong> \u2013 (left) these grafts originate on the aorta (main artery in the abdomen) and take blood to the femoral arteries at the groins. These are major operations with an approximately 3-5% (1 in 20) risk of dying. Aorto-bifemoral grafting is the most successful type of inflow operation, but is also the most traumatic. A Dacron graft (usually) is stitched to the aorta in the abdomen, just below the level of the kidneys. The graft has two legs which are then tunnelled under the abdominal muscles into the groins to be attached to the femoral arteries. A diagram of an aorto-bifemoral graft is shown on the left.<\/p>\n<p><strong>Axillobifemoral grafting<\/strong> \u2013 these grafts originate from the axillary arteries (supplying the arms) just below the collar bones and take blood to the femoral arteries. This is sometimes the best option in very elderly or very unfit patients as it is a less traumatic alternative to aorto-bifemoral grafting.<\/p>\n<p><strong>Femoro-femoral cross-over grafting<\/strong> \u2013 (below right) these grafts originate from a normal femoral artery in the groin on one leg and take blood to the femoral artery in the groin on the opposite leg. In the example shown below right a graft is running from the left femoral artery in the patient to the right femoral artery. At least one of the arteries running to the legs must be flowing normally in order to perform this operation.<\/p>\n<figure id=\"attachment_137\" aria-describedby=\"caption-attachment-137\" style=\"width: 226px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-137\" src=\"https:\/\/cmcdeploytwo.co.nz\/vsoanz\/wp-content\/uploads\/2025\/10\/Femoro-femoro-bypass-.jpg\" alt=\"Femoro-femoro bypass\" width=\"226\" height=\"150\" \/><figcaption id=\"caption-attachment-137\" class=\"wp-caption-text\">Femoro-femoro bypass<\/figcaption><\/figure>\n<p><strong>Ilio-femoral bypass grafting<\/strong>\u00a0\u2013 these grafts originate from the iliac arteries in the pelvis and take blood to the femoral arteries in the groins.\u00a0 These can be useful if there is enough normal iliac artery in the pelvis.<\/p>\n<p>The common theme with these operations is that they restore good blood flow to the femoral artery at the top of the leg in the groin region. Restoring the inflow to normal levels is often all that is required in many patients, even if there is disease in the arteries further down the leg.\u00a0 The improvement in inflow alone can be enough to permit healing or reduce symptoms to a more bearable level.<\/p>\n<p><strong>Outflow operations<\/strong>\u00a0If the arterial flow to the femoral artery is good and there are good femoral pulses in the groin, an inflow operation is not required.\u00a0 If the patient has a shortage of blood to the leg and good femoral pulses then the arterial problem must lie at a lower level in the thigh and\/or the calf arteries.\u00a0 These operations are frequently prolonged and complex procedures and best results are obtained by using vein grafts if possible.\u00a0 There are many operations to restore blood flow to the legs, but the commonest are:<\/p>\n<p><strong>Femoro-popliteal bypass<\/strong>\u00a0\u2013 these grafts originate from the femoral artery in the groin and take blood to the popliteal artery either just above or below the knee.<\/p>\n<p>The video opposite shows the main stages of femoro-popliteal bypass.\u00a0 If you are squeamish it may be better not to watch this video.<\/p>\n<p><strong>Femoro-distal or femoro-crural bypass<\/strong>\u00a0\u2013 these grafts originate from the femoral artery at the groin and take blood to one of the 3 calf blood vessels (anterior and posterior tibial arteries and peroneal artery).<\/p>\n<figure id=\"attachment_138\" aria-describedby=\"caption-attachment-138\" style=\"width: 340px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-138\" src=\"https:\/\/cmcdeploytwo.co.nz\/vsoanz\/wp-content\/uploads\/2025\/10\/Femoro-popliteal-bypass-.jpg\" alt=\"Femoro-popliteal bypass\" width=\"340\" height=\"202\" srcset=\"https:\/\/cmcdeploytwo.co.nz\/vsoanz\/wp-content\/uploads\/2025\/10\/Femoro-popliteal-bypass-.jpg 340w, https:\/\/cmcdeploytwo.co.nz\/vsoanz\/wp-content\/uploads\/2025\/10\/Femoro-popliteal-bypass--300x178.jpg 300w\" sizes=\"auto, (max-width: 340px) 100vw, 340px\" \/><figcaption id=\"caption-attachment-138\" class=\"wp-caption-text\">Femoro-popliteal bypass<\/figcaption><\/figure>\n<p>The picture above shows the appearance of the leg following a femoro-peroneal bypass.\u00a0 This patient also required amputation of toes.<\/p>\n<p><strong>Pedal bypasses<\/strong>\u00a0\u2013 these grafts can originate from any artery in the leg (femoral, popliteal and rarely tibial) and take blood to arteries below the level of the ankle joint (dorsalis pedis artery and posterior tibial artery or their branches).\u00a0 For pedal and distal bypasses the arteries are small and difficult to stitch with the naked eye so magnifying glasses are required to perform these operations reliably.<\/p>\n<p>Femoral endarterectomy is a similar operation to carotid endarterectomy in which the diseased lining of the artery can be removed and blood flow restored.<\/p>\n<p><strong>General graft principles<\/strong>\u00a0For inflow operations it is better to use artificial grafts.\u00a0 They perform well when the arterial flow is high and have been shown over years to provide durable results.\u00a0 The commonest material used for inflow operations is<a href=\"http:\/\/en.wikipedia.org\/wiki\/Dacron\" target=\"_blank\" rel=\"noreferrer noopener\">\u00a0Dacron<\/a>.<\/p>\n<p>For outflow operations it is generally better not to use artificial materials.\u00a0 They do not function as well in situations of lower blood flow in smaller arteries.\u00a0 In these circumstances it is better to use veins harvested from the leg as the conduit or tube to take blood (Pereira et al 2006). \u00a0These veins are also used in patients having heart bypass operations.\u00a0 Sometimes artificial materials have to be used as no vein is available and in these circumstances\u00a0<a href=\"http:\/\/en.wikipedia.org\/wiki\/Polytetrafluoroethylene\" target=\"_blank\" rel=\"noreferrer noopener\">Goretex (PTFE)<\/a>\u00a0is the commonest material in use, although there is some reasonable\u00a0<a href=\"http:\/\/ac.els-cdn.com\/S0741521410003058\/1-s2.0-S0741521410003058-main.pdf?_tid=d93135ae-8066-11e2-b499-00000aacb361&amp;acdnat=1361919179_0e4907e481e6523cf6c2fc5ece6841d5\" target=\"_blank\" rel=\"noreferrer noopener\">evidence<\/a>\u00a0that for femoro-popliteal bypasses Dacron impregnated with<a href=\"http:\/\/en.wikipedia.org\/wiki\/Heparin\" target=\"_blank\" rel=\"noreferrer noopener\">\u00a0heparin<\/a>\u00a0can perform better. (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/19231253\" target=\"_blank\" rel=\"noreferrer noopener\">van Det, 2009<\/a>) In general with any graft it is better to make the grafts as short as possible and to avoid crossing joints, but quite frequently, grafts have to be long and cross joints to salvage the leg and foot. The grafts are stitched to the arteries using fine polypropylene stitches (similar to nylon).\u00a0 Permanent stitches are used because the joins between the grafts and the native artery require lifelong support.\u00a0 This is especially important when using artificial grafts.<\/p>\n<p>&nbsp;<\/p>\n[\/vc_column_text][\/vc_tta_section][vc_tta_section title=&#8221;What types of anaesthetic are used?&#8221; tab_id=&#8221;1759369422435-7bce3ba6-e85f&#8221;][vc_column_text css=&#8221;&#8221;]\n<h2 class=\"wp-block-heading\">What types of anaesthetic are used?<\/h2>\n<p>Bypass surgery is frequently time consuming and complicated. General anaesthesia where the patient is asleep and unconscious throughout the operation is probably the commonest form of anaesthesia used. Sometimes epidural anaesthesia can be used either on its own or in conjunction with general anaesthetic. In this technique a fine tube is inserted into the tissues around the spinal cord and the nerves to the legs are frozen. This works very well in that the patient will not feel any part of the operation but it can be difficult to lie still for 3 or more hours. If vein needs to be havested from the arm, an epidural will not be effective alone.<\/p>\n<p>Frequently epidurals are used as well as a general anaesthetic. This is not only good for pain relief after the operation, but may also help to improve the blood flow in the graft by dilating the blood vessels in the leg. Local and regional anaesthesia can be used for bypass surgery in the legs, but it is much less common. It\u2019s main use would be in patients who are very unfit and at high risk from any other form of anaesthesia.[\/vc_column_text][\/vc_tta_section][vc_tta_section title=&#8221;SectWhat are the complications of bypass surgery?&#8221; tab_id=&#8221;1759369423744-c5f03bb4-cb2c&#8221;][vc_column_text css=&#8221;&#8221;]\n<h2 class=\"wp-block-heading\">What are the complications of bypass surgery?<\/h2>\n<p>Complications of any operation can be divided into local and general.<\/p>\n<p><strong>Local complications<\/strong><\/p>\n<p><strong>Graft failure<\/strong>\u00a0\u2013 the bypass graft may block and the blood supply to the leg and foot will be reduced to the levels that were present before the bypass surgery.\u00a0 In fact on many occasions when a bypass graft blocks, especially in the early period (hours to weeks) after surgery, the blood supply can be worse than before the bypass surgery.\u00a0 In these circumstances amputation can be inevitable unless the graft can be salvaged and the blood supply restored.<\/p>\n<p><strong>Wound infection<\/strong>\u00a0\u2013 wound infections are not uncommon in vascular bypass surgery in the lower leg.\u00a0 This is because the incisions are long, the operations are prolonged and the tissues of the leg are often already damaged and frequently swollen.\u00a0 Superficial wound infections are usually treatable by a combination of dressings, antibiotics and sometimes drainage of infection.<\/p>\n<p><strong>Graft infection<\/strong>\u00a0\u2013 if the infection involves the graft, especially if the graft is PTFE or Dacron, the situation is very much more serious.\u00a0 It is frequently impossible to eradicate infection from artificial materials and they need to be removed.\u00a0 When this happens the blood flow down the bypass graft is also removed.\u00a0 Unless an alternative route for a bypass graft can be found, this situation will frequently end in amputation.\u00a0 Graft infection is fortunately uncommon, probably only affecting about 5% of prosthetic grafts.\u00a0 The presence of infection around a vein graft is more controllable and less likely to end in serious consequences.<\/p>\n<p>One particular type of graft infection can be critical for any type of graft.\u00a0 In this infection the surgical join (anastomosis) between the artery and the graft is involved.\u00a0 If this has occurred there is a risk of serious bleeding from the join as the seal becomes weakened by the infection.<\/p>\n<p><strong>Leg swelling<\/strong>\u00a0\u2013 this is a very common event after lower limb bypass surgery.\u00a0 It is due to a combination of factors.\u00a0 Much of it may resolve, but frequently persistent swelling is the price a patient pays to avoid amputation.<\/p>\n<p><strong>General complications<\/strong>\u00a0Chest infections, heart attacks and strokes \u2013 because bypass surgery is prolonged and frequently takes place in elderly patients with other significant problems, these complications are not uncommon.\u00a0 However, they are just as common, if not more so, in patients undergoing amputation which is frequently the only alternative.[\/vc_column_text][\/vc_tta_section][vc_tta_section title=&#8221;Will I need further treatment after my operation?&#8221; tab_id=&#8221;1759369501070-83ea97ce-c713&#8243;][vc_column_text css=&#8221;&#8221;]\n<h2 class=\"wp-block-heading\">Will I need further treatment after my operation?<\/h2>\n<p>All patients with hardening of the arteries should take a small dose of aspirin every day. As little as 75 milligrammes of aspirin per day is beneficial and should be continued after your operation. Aspirin interferes with the function of platelets in the blood. Platelets are important in blood clotting and aspirin \u201cthins\u201d the blood making clots less likely to form.\u00a0 A new medication \u2013 clopidogrel (Plavix) \u2013 is being used more frequently.\u00a0 It has little advantage over aspirin alone but there is increasing evidence that the combination of aspirin and clopidogrel may be more beneficial than either alone. In some patients aspirin alone is not considered a strong enough treatment to prevent blood clotting. In these patients warfarin can be used either alone or with aspirin to reduce the chances of clots. If you are taking warfarin tablets it is important that you have regular blood coagulation tests. These monitor how well the warfarin is working and ensure that it is kept under control. There is some evidence that use of warfarin prolongs the patency of bypass grafts in the legs but for various reasons it is not used as frequently as it probably should, possibly because of concerns over bleeding. It is probably most beneficial in smaller diameter (6mm) prosthetic grafts\u00a0<a href=\"http:\/\/ac.els-cdn.com\/S0741521402486569\/1-s2.0-S0741521402486569-main.pdf?_tid=5a20dfd0-8066-11e2-a1d4-00000aab0f27&amp;acdnat=1361918966_edf846e6e585ed36264dd2a34c94e899\" target=\"_blank\" rel=\"noreferrer noopener\">(Johnson et al 2002<\/a>). In addition to aspirin your vascular risk factors should be treated.<\/p>\n<p><strong>Duplex surveillance<\/strong>\u00a0In about one third of grafts performed with veins a narrowing develops after the operation. To identify patients who develop narrowing, Duplex scanning of the graft is usually recommended at frequent intervals over the first two years.\u00a0 This enables problems to be identified at an early stage when they can be corrected before the graft blocks.\u00a0 Once a graft blocks it is usually more difficult to rescue the situation and restore normal blood flow.\u00a0 This sort of scanning is most useful in patients with vein grafts.[\/vc_column_text][\/vc_tta_section][vc_tta_section title=&#8221;What are the results of bypass surgery?&#8221; tab_id=&#8221;1759369499668-a9431759-1977&#8243;][vc_column_text css=&#8221;&#8221;]\n<h2 class=\"wp-block-heading\">What are the results of bypass surgery?<\/h2>\n<p>The results of bypass surgery vary enormously depending on the the type of graft (artificial or vein graft) inserted, the severity of the arterial disease and the site of the graft. The larger grafts taking blood to the femoral arteries at the groin have good patency rates of 70-90% functioning normally at 5 years after surgery. In femoro-popliteal grafts above the knee about 80% of vein grafts will be working and patent 5 years after surgery.\u00a0 In femoro-popliteal grafts below the knee about 65% of vein grafts will remain patent 5 years after surgery.\u00a0 Results for artificial grafts (PTFE) are worse.\u00a0 For above knee PTFE femoropopliteal bypass grafts about 60% will be working at 5 years (<a href=\"http:\/\/ac.els-cdn.com\/S0741521406008214\/1-s2.0-S0741521406008214-main.pdf?_tid=f04eef48-8065-11e2-ab9e-00000aab0f02&amp;acdnat=1361918788_c7a82148d79fa57ba344e5fc9f1d494a\" target=\"_blank\" rel=\"noreferrer noopener\">Pereira et al 2006<\/a>).\u00a0 Results for more complex procedures to the calf vessels are usually slightly worse than these figures. Femoro-femoral bypass grafts are durable with patency rates at 5 years between 74-90%.\u00a0 Early complications occurred in approximately one fifth of these patients within the first 30 days after surgery (<a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/15810055\" target=\"_blank\" rel=\"noreferrer noopener\">Pursell R et al, 2005<\/a>).<\/p>\n<p>A recent study (<a href=\"http:\/\/www.intromedix.co.il\/loadedFiles\/Adam_BASIL_Lancet%202005.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">BASIL, 2005<\/a>) has compared angioplasty with bypass surgery in patients with severe limb ischaemia.\u00a0 In patients who are suitable for both angioplasty\u00a0<strong>and\u00a0<\/strong>bypass surgery, an angioplasty first strategy had broadly similar outcomes to surgery.\u00a0 Unfortunately, the majority of patients presenting with severe disease (approximately 80%) are not suitable for both strategies and are more likely to require surgery.\u00a0 In general vascular specialists will attempt angioplasty first in place of surgery if there is a reasonable chance of a good outcome.\u00a0 If this fails bypass surgery is still likely to be an option.\u00a0 Later results in this highly selected group of patients do show a benefit for surgery in terms of reduced risk of amputation, but only if the patient was alive 2 years after surgery.<\/p>\n<p>Finally it was previously thought that bypass surgery, especially infrainguinal bypass surgery, by improving the arterial blood supply to the leg would also enable patients to become more mobile and improve functional outcomes. Personal experience and the literature (<a href=\"http:\/\/education.surgery.ufl.edu\/Lectures\/RecoveryafterBypass.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Nicoloff et al, 1998.<\/a>) indicates otherwise. A patient who comes into hospital unable to walk is unlikely to walk again after a bypass operation.
